If you would need 4 periodontal cleanings, instead of 3, the fee is $875 per year.

No. This membership is not dental insurance. It is an in-office preventative care and savings program available exclusively at our practice. It cannot be combined with dental insurance or other discounts.

Your membership includes the specific services listed in your selected plan for a 12-month period starting from your enrollment date. Services must be used while your membership is active and do not roll over if unused.
Any treatment not listed in your plan will be billed separately.

No. This membership program is not available to patients who have dental insurance and cannot be combined with any insurance plans, discount programs, or third-party coverage.

Yes. Active members receive 20% savings on select retail products, including electric and manual toothbrushes, toothpaste, whitening products and other products available in our retail section.
Excludes Oral B electric toothbrushes, Hydrofloss and professional whitening trays.

Membership fees are due at enrollment and are non-refundable and non-transferable. Membership benefits begin on the enrollment date and cannot be applied retroactively to past visits, treatments, or purchases.
New patients may enroll on the day of their first appointment and begin using benefits at that time. Services must be used while the membership is active and do not roll over if unused. Services not included in the plan will be billed separately.
New patients will receive a CBCT scan at their first appointment, which is included in the new patient membership plan. After their first year they can switch to the existing patient plan. Existing patients receive a discounted membership rate; if they are not due for a new CBCT, the CBCT benefit may be saved and used later in the event of a dental emergency during the active membership period. Patients may use their “emergency exam’’ during their second cleaning of the year.

Payment of the membership fee constitutes acceptance of all Terms and Conditions outlined above.
